Step 1 - Complete the Application & Payment
Please fill out the form in its entirety and include all required documentation.
Step 2: Provide Access to the Course
In order for HCA to complete the review process, we require temporary full access to your program:
If your program is online, please provide a username and password and grant full access to the entire course.
For physical programs, live summits, conferences please provide full transcipts in digital or hard copy form.
Books (hardcopy preferred) can be submitted by mailing them to:
